Mechanism of Action
Zolpidem selectively binds to the alpha-1 subunit of the GABA-A receptor (BZ1 receptor). Unlike benzodiazepines (which bind non-selectively to multiple GABA-A subtypes), Zolpidem's selectivity was designed to produce pure hypnotic (sleep-inducing) effects without significant anxiolytic or muscle relaxant properties.
Why It Is Abused
If the user takes Zolpidem and deliberately fights the urge to sleep, it produces a trance-like, disinhibited, euphoric state with visual distortions and a feeling of detachment from reality. It is also combined with alcohol for an amplified effect.
Side Effects & Dangers of Abuse
- Complex Sleep Behaviors: Sleepwalking, "sleep-driving," sleep-cooking, and sleep-eating while not fully conscious. These behaviors carry an FDA black box warning.
- Amnesia: Complete memory blackout of events during the intoxicated period.
- Dependence: Rapid tolerance development and a withdrawal syndrome similar to benzodiazepines (rebound insomnia, anxiety, seizures).

Mechanism of Action
Zopiclone acts on the same GABA-A benzodiazepine receptor complex but at a different molecular site than classical benzodiazepines. It has a rapid onset and intermediate duration (half-life 5-6 hours).
Why It Is Abused
Similar to Zolpidem, Zopiclone is abused by those who take it and resist sleep. It produces a metallic taste in the mouth (a pharmacological signature) alongside sedation and euphoria. It is frequently combined with alcohol.
Side Effects & Dangers of Abuse
- Metallic/Bitter Taste: A unique and persistent side effect.
- Next-Day Impairment: Hangover effects impair driving and cognitive function.
- Dependence: Though marketed as "less addictive" than benzodiazepines, chronic abuse leads to significant physical dependence.
3. Nitrazepam

Mechanism of Action
Nitrazepam is a classical benzodiazepine hypnotic. It enhances GABA-A activity with a prolonged half-life (15-38 hours), making it highly sedating.
Why It Is Abused
Nitrazepam is one of the oldest and most well-known sedatives. It is abused for its powerful, long-lasting sedation. In polysubstance abuse, it is combined with alcohol or opioids for an amplified CNS depression.
Side Effects & Dangers of Abuse
- Extreme Oversedation: Due to its long half-life, the patient may remain impaired for 24+ hours.
- Falls in the Elderly: Beers Criteria explicitly warns against its use in older adults.
- Teratogenicity: Risk of birth defects if taken during pregnancy.
